Rather than make a name placard for myself, I decided to make one for my girlfriend to use at her new job at Google.

The base is made of several different colors of acrylic glued together. There are several holes cut in all but the bottom layer that allow three things to stand up - the actual name, a second clear piece that says Google (and is replaceable in case she changes jobs ;-)) and a hole to hold business cards.
The trickiest thing was getting the size of the slit correct so that the acrylic piece with her name on it would slide in easily - I ended up doing this by cutting the first base piece several times until it fit correctly, then using that version of the file to cut the remaining base pieces.
I ended up only using a small piece of vinyl since I didn't think it looked very good - I actually don't have a picture of it but the business cards I put in the name tag originally were vinyl.
